Some further digging...
I took a look at the WR speed run. Almost every attack is that high damage run-jump-downward-sword move, timed perfectly to hit the enemy every time they stand up. They took no damage throughout. Still ended up with 247 score, 277 strength and class A+++, exactly the same ratings as your play through 😀
GameFAQs says that the class rating (A+++) is just a letter representation of your Strength score. If you get over 100 strength then its going to be A+++ always. And the number beside the Class is a counter for that session:
"The numbers on the right-hand side of the table indicates the number of times you have achieved that grade during your current sitting of the game."
https://gamefaqs.gamespot.com/genesis/563326-golden-axe/faqs/60348#section28
So if you beat it you are always going to be 247, 277, A+++. And the counter will vary from 01, 02, 03, etc depending on how many times you repeated that session.
The time you got A++ 02 without completing it was probably the 2nd run of that session. And you dropped a + for dying before the end.

Yea. For game duel completion it seems everyone gets the same metrics for everything. I wasn't sure until now but the others do too. But that was a good thought.
I jumped kicked the entire time. It would have been neat to see higher scores for pulling off more variety or getting hit less or somethin. This is just one of those games that max out

Yeah I was wondering about the Strength score too. I was thinking that if 247 was the same for a lot of runs, then perhaps one of the other scores could be used instead, to better distinguish the runs and decide a ranking order instead of all joint first place. But its not overcrowded with 1st places just yet, and the other metrics are not too clear, so its all good.

That's a good question. Judging by the other top scores in the emulation section, total score 247 seems the highest you can get and is the same for completing everything.
But as far as the strength number and class number I don't know.
It says I have 277.0 strength and idk what measures that.
And it says I'm 100 ~ A++ 01 Class.
And below that is 00.
One time I got a 100 ~ A++ 02 Class, but didn't beat the final enemies.
And I know 02 is higher than 01 because it shows 00 below the 01.
Lol I know I just made things 10x more confusing. I'm not sure how it calculates any of it.
The other players got a 277 strength for beating it as well

Just curious, what is the scoring system for The Duel? Is the score based on how quickly you beat it, or health remaining, or will you always end up with 247 once you complete it?

I saw Siliconian play some duel tonite, I remembered I had beaten it in the past, so figure it was worth another go for a lvlup record. Took me 3 plays and wow is my left hand tired. After play 1 my thumb went out.. so I set the controller on my lap and used my fingers on the dpad. Worked nicely.
